Full time Case Manager position available in Leominster.

Key responsibilities include building trusting relationships with individuals in recovery. Providing mentorship. encouragement, and navigation through systems of care. Assisting individuals in creating recover plans and identifying goals related to health, wellness, housing, employment, and social connectedness. Accompanying individuals to appointments and advocating with or on behalf of them when needed. Collaborating with clinical teams, community partners, and local agencies including courts, detox centers, shelters, hospitals, and community programs to support client care.
A four-year or higher education degree in human services, psychology or related field is required. Must be in personal recovery for 2+ years and have a strong understanding of receover priciples and pathways.
Excellent communication, organizational, and problem solving skills. Comfort with technology and electronic record keeping. Must have reliable transportation and valid driver's license for field based work.
Maedon moved to Leominster from the Washington, DC area in 1986. She joined the family business in 1991 and purchased Appleseed when her mother retired in 1997. Maedon is actively involved in the community, sitting on the board of directors for Nashoba Valley Chamber of Commerce, Montachusett Recovery Center in Leominster, and the advisory board chair at Crystal House in Gardner. Maedon is passionate about the work she does, both in and out of the office.
She is the mother of two adult sons and “Brabra” to her favorite people, three young granddaughters. She makes sure there is always time for family and friends, travel, her flower gardens, and her Great Dane Hazel.
